1. How old is the child?

2. Which level in school is the child in?
3. Does the child have any diagnosed learning difficulties?
4. Does the child have any specific subject or areas that require help?

5. Are you looking at home-based therapy or can you send the child to the place of remediation? (If you prefer home-based therapy, may we have your location?)
